Elaboración de dietas individualizadas

Para conseguir los objetivos nutricionales concretos de un individuo las dietas individualizadas son las más apropiadas y minuciosas ya que se ajustan a las características y a la situación de cada sujeto. Se debe recopilar mucha información sobre el individuo antes de comenzar a elaborar la dieta para hacerlo de la manera más exhaustiva posible. Para alcanzar dichos objetivos y pautar una dieta se debe tener información personal del paciente, como la edad, el sexo, gustos, restricciones alimentarias, si ha seguido dietas anteriormente y sus objetivos; se debe tener en cuenta el grado de salud y la historia clínica, para incluir únicamente los alimentos adecuados para su estado de salud y ajustar la dieta a la mejora o prevención de enfermedades; su actividad física, con el fin de conocer su gasto calórico habitual para ajustar la ingesta calórica y proporcionar sugerencias para compatibilizarla con la dieta; y por último, los datos de un análisis hematológico y bioquímico, para conocer el estado del paciente y detectar posibles alteraciones que puedan ser mejoradas con la dieta. También se debe realizar un estudio antropométrico con el objetivo de evaluar la situación del paciente mediante parámetros como el índice de masa corporal y poder realizar un seguimiento a lo largo del periodo que dure la dieta. Toda esta información se toma como punto de partida para poder empezar a plantear la dieta. Esta se debe ajustar a las necesidades del individuo y priorizar la preservación o la mejora de la salud y para ello debe proporcionar todos los nutrientes necesarios y en las cantidades adecuadas. Además es recomendable sugerir la habituación a realizar ejercicio físico y la propuesta de otras actividades para aumentar la actividad física total a fin de apoyar la dieta y crear en el paciente rutinas saludables. Una vez iniciada la dieta por el sujeto se debe hacer un seguimiento de su evolución, con parámetros antropométricos, y de su opinión para tratar de evaluar si la dieta se adapta al paciente para mejorarla en los aspectos que considere peores o más difíciles de seguir y hacer que sea lo más agradable posible para conseguir que el paciente la siga lo más fielmente posible y que lo considere como una oportunidad para establecer nuevas costumbres que se puedan mantener a largo plazo. Al finalizar el periodo establecido para la dieta o al alcanzar los objetivos se debe volver a realizar una evaluación de los parámetros antropométricos para comprobar que se han alcanzado los objetivos de una manera apropiada y se recomienda un nuevo análisis hematológico y bioquímico si su médico lo considera conveniente. Si se tiene la oportunidad y así lo desea el paciente, es conveniente realizar una nueva evaluación pasado un tiempo después de la finalización de la dieta para comprobar que no vuelve a la situación de partida y, en tal caso, modificar la dieta, la actividad física o el planteamiento de cambios de hábitos. En este este trabajo, se ha escogido a tres individuos voluntarios de distintas edades y sexos, con distintos puntos de partida y objetivos con el fin de elaborar una dieta individualizada para cada uno de una duración de cuatro semanas. Se realizó el estudio previo de cada uno de ellos y según toda la información recopilada se elaboraron cuatro dietas semanales. Al finalizar cada semana se hizo un seguimiento teniendo en cuenta el peso y la opinión de cada uno para adaptar la siguiente dieta semanal a los resultados y a las apreciaciones hechas por los voluntarios. Al finalizar la dieta se evaluó el proceso completo y los resultados y se planteó una guía para que los pacientes fueran autónomos y continuaran con los nuevos hábitos creados.
In order to achieve the specific nutritional objectives of an individual, individualized diets are the most appropriate and thorough as they are adjusted to the characteristics and situation of each subject. A lot of information should be collected about the individual before starting to develop the diet to make it as thorough as possible. In order to achieve these objectives and to plan a diet, personal information should be obtained from the patient, such as age, sex, tastes and dietary restrictions, whether he or she has followed diets in the past and their objectives; the degree of health and the medical history should be taken into account, to only those foods that are appropriate for his or her state of health and to adjust the diet to improve or prevent disease; their physical activity, in order to know their habitual caloric expenditure to adjust the caloric intake and to provide suggestions to make it compatible with the diet; and finally, the data from a haematological and biochemical analysis, to know the state of the patient and to detect possible alterations that can be improved with the diet. An anthropometric study should also be carried out in order to evaluate the patient´s situation using parameters such as body mass index and to be able to follow the patient throughout the duration of the diet. All this information is taken as a starting point to be able to start planning the diet. This diet must be adjusted to the needs of the individual and prioritize the preservation or improvement of health and for this purpose it must provide all the necessary nutrients and in adequate quantities. It is also advisable to suggest the habituation to physical exercise and the proposal of other activities to increase the total physical activity in order to support the diet and healthy routines in the patient. Once the diet has been initiated by the subject, a follow up should be made of his/her evolution, with anthropometric parameters, and his/her opinion to try to evaluate if the diet is adapted to the patient, to improve it in the aspects that he/she considers worse or more difficult to follow and to make it as pleasant as possible to get the patient to follow it as faithfully as possible and to consider it as an opportunity to establish new habits that can be maintained in the long term. At the end of the period established for the diet or when the objectives are reached, an evaluation of the anthropometric parameters should be carried out again to check that the objectives have been reached in an appropriate way and a new haematological and biochemical analysis is recommended if his/her doctor considers it appropriate. If there is an opportunity and the patient so wishes, it is advisable to make a new evaluation some time after the completion of the diet to ensure that it does not return to the initial situation and, if so, to reconsider the diet, physical activity or the approach to changing habits. In this project, three volunteers of different ages and sexes, with different starting points and objectives, have been chosen in order to elaborate an individualized diet for each one of a duration of four weeks. The previous study of each one of them was carried out and according to all the information gathered, four weekly diets were elaborated. At the end of each week, a follow-up was made taking into account the weight and opinion of each one in order to adapt the following weekly diet to the results and to the appreciations made by the volunteers.
